We’re looking for just the right person to join our team as Leader, CONNECT Partners, a brand new company. • Service plans leverage neuroplasticity by pursuing resident goals in real-life, non-institutional settings;
• A “doing-with” coaching approach with blended job roles and a multi-professional coaching team.

You will support the team delivering services while keeping an eye on quality and best practices. You will guide the development of integrated services, ensuring a culture of collaboration and partnership exists between the individuals we support, their coaching resource team and community services providers.
